Simply buy 4 boxes and mail in rebate form (included with shipment), all box ends, and a copy of receipt. Rebate check will be from Bausch and Lomb.

4 boxes @ $42.00 = $168.00 - $40.00 = $32.00 per box after rebate.

Contact lens rebates are limited to one rebate per person per 12 month period.

Lenses with an AXIS other than 90 or 180 will have a delivery time of about one week.
